As a Department of Health Alcohol and Drug Abuse Division (ADAD) contracted treatment provider, you are part of the Hawai‛i CARES network to support healing and recovery for individuals struggling with Substance Use Disorder (SUD) across Hawai‛i. The Aloha United Way 211 SUD CARES program is the managing entity for the Hawaii SUD Continuum of Care to provide information and screening for Hawaii residents seeking SUD treatment and recovery services.

The SUD Care Coordination team uses a thorough and thoughtful screening process to collect and identify each individual’s needs, history/frequency of substance use, health/mental health concerns, and other information that will be used to determine eligibility requirements for a referral to a SUD program within the ADAD provider network.

Coordinators handle service utilization requests, contract reviews for appropriate billing, transitions in care, and ensure complete electronic records. Data from this shared system helps improve care statewide.

This consent form allows providers to access medical history and comprehensive client information is accessible across the CARES network. Please be sure to complete this form during the intake process.

This guide introduces using the HI-WITS telehealth system to ensure that providers can log in and correctly enter information.
